New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both of these forms can be found on the above "Registering Online" tab.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Named Accountable GP

We have been requested by the government under the terms of the latest GP contract to allocate all patients a named GP. 

All patients will be informed of their named GP.  At Dr Hallikeri and Partners, their named GP will be Dr Hallikeri.  If another GP has been allocated to you then you will be informed of your named GP. 

Your named accountable GP will be written on your medical card.  This does not mean you will require to see the named GP, you can see any GP which is employed by the practice.  

Changes to Personal Details

If you should change any personal or contact details please infirm us as soon as possible. You can download the relevant form here.
